If you are unable to submit an online query due to a disability, please speak to reception, who can submit the query on your behalf.
Please note that for new sicknote without documentation (e.g. Hospital discharge summary) or where review of you condition is required, you may be contacted by the surgery with self booking appointment link to asses your request.
